MGH Chelsea Community Health Improvement
Since 1997, Sarah has worked to bring health care equity to patients at the MGH Chelsea HealthCare Center. She leads a team of 45 community health workers who implement programs and strategies addressing social determinants of health, social services, immigrant and refugee health, family and community violence, hunger, and cancer prevention. Sarah also leads the Partners Community Health Worker Collaborative, which is tasked with implementing effective community health worker programs across Massachusetts, based on the 20 year success of the MGH Chelsea CHW model. She holds an MSW from Columbia University.
